The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend, facial coverings, social distancing limitations and the limits of congregation of groups of people to help prevent the spread.

In proceeding to register for and/or attend this event, I, and on behalf of those attending with me (collectively "we"), acknowledge the contagious nature of COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. We further acknowledge that the City of Centerville has put in place preventative measures to reduce the spread of the COVID-19. We further acknowledge that the City cannot guarantee that we will not become infected with the Covid-19. We understand that the risk of becoming exposed to and/or infected by the COVID-19 may result from the actions, omissions, or negligence of ourselves and others. We hereby release and agree to hold the City harmless from, and waive on behalf of ourselves, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the city, or that may otherwise arise in any way in connection with the City of Centerville. We understand that this release discharges the City of Centerville from any liability or claim that we, our heirs, or any personal representatives may have against the City with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise.
